AbstractAt present, there are ten civil transport airplanes operating with fly-by-wire (FBW) flight controls. The accident and incident record of these airplanes is reviewed based on publicly available accident or incident reports. Twenty-eight accidents were identified in which the FBW system played a part. The most common type of occurrence was an un-commanded pitch or bank. Other incidents involve pilot-induced oscillations or abrupt maneuvers. We will discuss some of the factors behind seven selected accidents and incidents. Issues discussed include fault detection and isolation, dispatch with inoperative components, envelope protections issues, inceptor arrangements, and pilot-induced oscillations. In spite of this focus on FBW incidents and accidents, we must not forget that overall, FBW and envelope protection have prevented accidents and saved lives.
